How We Categorized Our Findings
We have rated the devices using the following categories:
Efficacy
Efficacy of the devices was evaluated by comparing baseline pre-treatment home sleep test results to home sleep test results performed with the oral appliance. There were no statistical differences among the different types of appliances at the time the evaluation was performed*.
Comfort
Devices are rated on comfort based on patient feedback. Although comfort is subjective, it can also be influenced by a patient’s unique dental anatomy. Certain dental conditions may make one style of sleep device more comfortable than others.
Maintenance
We rated each oral appliance by ease of use to clean and maintain, and considered the cleaning products which may be needed.
Durability
We rated each device on how it handles normal wear and tear, and reviewed certain parts of the devices prone to damage.
Warranty
Warranties differ between manufacturers and device types. We outlined each device’s warranty below. We only prescribe devices that include a warranty. Device warranties cover normal wear and tear. This does not include loss, theft, or pet damage.
Advancement
Oral appliance therapy moves the lower jaw slightly forward. However, the lower jaw may need to be further forward to sleep your best. Each device can be advanced to bring the jaw further forward in small increments.This process is called titration. Sleep devices are rated by the style of titration and how much it can be titrated.

The Panthera classic is medical grade nylon which is strong, thin, and moderately flexible. Easily adjustable in 0.5mm increments using interchangeable rods. Significant titration range of 16-34mm. This style is gentler on the jaw/TMJ; can be customized for TMJ disorders; and has rubber band hooks to keep the mouth closed, promoting nasal breathing. It’s easy to clean. Soak all day in Novadent solution. The 3 year warranty covers basic wear and tear.
ProSomnus® EVO®

The EVO™ is a 3D milled medical grade plastic. It is exact fitting, highly resistant to staining, and has some flexibility. It fits like a tight retainer and works for short teeth. The initial set up allows for 5 advancements and more can be ordered free of charge. This device can be customized to add rubber bands which promotes nasal breathing. The 3 year warranty covers basic wear and tear.
ProSomnus® [PH]

The Prosomnus [PH] is a firm, milled acrylic. It won’t be loose even on short teeth. It precisely advances up to 5 mm in 0.1 mm increments. This style is gentler on the jaw/TMJ; can be customized for TMJ disorders; and has rubber band hooks to keep the mouth closed, promoting nasal breathing. The 3 year warranty covers basic wear and tear, but the Herbst bars are not included.
DynaFlex Herbst®

The Dynaflex Herbst® is a fixed-hinge, 3D milled acrylic with an inner liner that easily adjusts following minor dental work like fillings. It precisely advances up to 5 mm in 0.1 mm increments. This style is gentler on the jaw/TMJ; can be customized for TMJ disorders; and has rubber band hooks to keep the mouth closed, promoting nasal breathing. The 1 year warranty covers basic wear and tear, but the Herbst bars aren’t included. An additional 3 year warranty can be purchased for a fee.
Glidewell Silent Nite®

The Glidewell Silent Nite® is a flexible thermoplastic material and one of the most comfortable devices on the market. It’s the best choice for those undergoing major dental work; who may need a new device in the future; and works well for small mouths. It advances in 1 mm steps with blue slide link bands. This device may become loose over time but is covered with a 2 year warranty.

Dr. Jackson is an NYU College of Dentistry graduate, and was a resident in a Level I trauma center. She is a Diplomate of the American Board of Dental Sleep Medicine. She worked as a general dentist for 10 years focused on digital dentistry, CBCT cone beam guided surgeries, CEREC crowns and onlays.
